Fall 2024 REGISTRATION INFORMATION
Please consider volunteering as a head coach. We are always needing head coaches to accommodate the number of new players joining FSA, especially in 8U and younger age groups.
Once registered, the next contact will be from the Coach approximately 2 -3 weeks prior to the start of the season
(Fall season-END of Aug/EARLY Sept, Spring season-END of Jan/EARLY Feb).
Be sure to have either your player's birth certificate or passport available to upload during registration, ALL PLAYERS WILL BE REQUIRED TO UPLOAD VERIFICATION OF BIRTH AND FORNEY SPORTS PASS!

Please remember that attendance at both practices and games is a very important part of the program. We cannot schedule around other leagues so if your player is participating in multiple sports/activities, we would ask that you make your soccer team your priority in the event of conflicts.

REQUIREMENT FOR NEW PLAYERS
Copy of birth certificate is required before registration is complete.
Proof of Birth Forms Accepted:
Birth Certificate-see additional information if a player has a foreign birth certificate
Passport
(No Hospital/Religious records will be accepted)
Email a copy of the birth certificate to the FSA Registrar

How to Upload a Picture
ALL Players must upload a photo – returning or New to FSA
Photo - Head Shot Only (Not a Body Shot)
· Action Shots will NOT be accepted
· Photo must be "Current" and "Clear"
- No Group Shots
Players must upload a photo to be used on an ID card. Upper body pictures only. No group photos or action shots will be accepted.

IMPORTANT INFORMATION FOREIGN BIRTH CERTIFICATES
INTERNATIONAL CLEARANCE PROCESS
Any player over the age of ten (10) who has a birth certificate from another country must complete the International Clearance through FIFA. Forney Soccer cannot put players onto teams who have not completed this paperwork. It is recommended that you start this process right away to avoid the player missing games. US Soccer estimates it could take as much as two months to process applications.
Players under the age of 10 with a foreign birth certificate are encouraged to apply right away to avoid delays when they reach age 11.

POLICY
An individual recreational player will receive a full refund if the WRITTEN request is made before placement begins. After placement and up until the season begins a $20 fee will be deducted from the refund. After the season begins no refund requests will be accepted.
